Insurance authorization

Our Insurance referral specialist can help you obtain the necessary referral authorization from your insurance company.

There are a few helpful hints to follow:

It is important for you to notify your primary care physician that you are coming to see one of our doctors at Naugatuck Valley Gastroenterology Consultants. You will need a separate referral for each visit or procedure unless your first referral specifies follow-up visits or procedures.

You may need to pick up a paper referral or an authorization number will be faxed to us. Ask your primary care physician’s office which situation applies to you.

When you contact them, they will need the following information:

  1. Your insurance
  2. Date of visit
  3. Which physician you will be seeing
  4. What service is to be done
    1. New patient visit
    2. Follow up visit
    3. Endoscopy (EGD, this is the stomach scope)
    4. Colonoscopy, (full colon scope)
    5. Esophageal Manometry or 24 hour Ph monitoring
  5. At which facility you are to be seen
    1. Our medical office
    2. Naugatuck Valley Endoscopy Center
    3. Naugatuck Valley Surgical Center
    4. Waterbury Hospital Gastroenterology Lab

Try to give your Primary Care Physician’s office at least 7 – 10 working days before your appointment to allow time for the following.

  1. Your primary care physician must first OK your visit
  2. Then the referral request is called in or faxed to your insurance company
  3. Then a doctor or nurse reviews your case and then either allows or denies your treatment.

IT IS POSSIBLE FOR YOUR VISIT TO BE DENIED BY YOUR INSURANCE COMPANY EVEN IF OUR DOCTORS AND YOUR PRIMARY CARE DOCTOR AGREE THAT IT SHOULD BE DONE.

are participating with your insurance plan. You will need to check your Provider Directory (you should have received one from your insurance company) or you can call provider relations, the number will be on your insurance card.
